By the end of this guide, you'll have a systematic ChatGPT workflow for prior authorization research and appeal letter writing that cuts your per-PA time from 45–90 minutes to 10–15 minutes. You'll use ChatGPT Plus to research medical necessity criteria, identify relevant clinical guidelines, and generate a complete, professionally written appeal letter ready for submission.

Create a system prompt you'll reuse for every PA. In a new conversation, paste this:
You are a clinical pharmacy specialist helping write prior authorization appeal letters. When I give you a drug name, indication, denial reason, and patient history, you will:
1. Identify the medical necessity criteria this payer typically requires for this drug
2. Research the relevant clinical guidelines that support the treatment (ADA, ACC, ASHP, IDSA, etc.)
3. Write a complete, professional PA appeal letter with:
- Clear medical necessity statement
- Patient-specific clinical justification (using the facts I provide)
- Reference to clinical guidelines with specific recommendations
- Documentation of treatment failure or contraindications to alternatives
- A specific, actionable request
Format the letter professionally with pharmacy letterhead placeholder, date, patient identifier, and signature line.
In the same conversation, paste the denial information:
Drug: [Drug name and strength]
Patient diagnosis: [Primary diagnosis and relevant comorbidities]
Denial reason: [Exact denial reason from payer notification]
Step therapy required: [What prior medications the payer claims are required]

ChatGPT will generate a complete appeal letter draft. Review it for:

Review the complete letter, edit any clinical details, and have the pharmacist-in-charge review before submission. Submit via CoverMyMeds, fax, or the payer's online portal.
GLP-1 prior auth for type 2 diabetes:
PA appeal for [GLP-1 drug] for type 2 diabetes. Denied for step therapy (metformin required). Patient has been on metformin [dose] for [duration] with A1C [value]. Include ADA 2025 guideline recommendations for early use of GLP-1 agonists in T2DM.
SGLT2 inhibitor for heart failure:
PA appeal for [SGLT2 inhibitor] for heart failure. Include 2022 AHA/ACC HF guideline recommendations for SGLT2 inhibitors in HFrEF/HFmrEF, and patient's EF and hospitalization history.
Specialty biologic for inflammatory condition:

Controlled substance for chronic pain:
PA appeal for [controlled substance] for [pain indication]. Include documentation of multimodal treatment failure, functional impact, and relevant prescribing guidelines for appropriate controlled substance use.
